New programs available through Teladoc Health

Diabetes Management Program

TeamCare’s newly offered program for Type I and Type II diabetics through Teladoc Health gives you the tools to learn how to set realistic healthy goals and stay committed.

The Diabetes Management program offers:

• Access to a dedicated team of providers, registered dieticians, health coaches, and mental health coaches.
• Supplies needed to help you manage your diabetes including a blood glucose meter, test strips, and lancets—all at no cost to you!
• Real-time feedback, personalized lessons, action plans, and messages from your team through connected devices.

If you are age 18 or older and have been diagnosed with diabetes, Teladoc Health will be sending you a letter on how to enroll in the Teladoc Health Diabetes Management program.

 

Mental Health Complete Program

Currently, TeamCare members can schedule visits with board-certified psychiatrists and licensed therapists for treatment of mental health conditions such as anxiety, depression, and stress.

TeamCare members will have access to additional mental health support between visits including:

• On-demand digital messaging with a certified mental health coach.
• Access to digital content such as self-guided programs.
• Online tools are available 24/7 through the Teladoc Health app or member portal—securely, discreetly, and confidentially.
